Pancreatic Cysts

Risk-stratified assessment of pancreatic cystic lesions using imaging, EUS and selective surgical management.

Not all pancreatic cysts are the same

Pancreatic cystic lesions include several entities with very different biological behaviour. Some require only surveillance, while others have features that may justify EUS assessment, tissue/fluid analysis or surgical discussion.

How cysts are assessed

MRI/MRCP and high-quality CT define morphology, ductal anatomy, size and associated features. EUS can provide additional structural information and, in selected cases, facilitate sampling when the result is likely to alter management.

The clinical question

The key decision is not simply “is there a cyst?” but whether its type and features indicate low-risk surveillance, closer investigation or consideration of resection.

When surgery is considered

Surgery is reserved for lesions in which the estimated malignant potential and patient-specific risk justify pancreatic resection. Age, comorbidity, lesion location and the type of operation required all matter.
